Mathieu van der Poel won stage 4 of Tirreno Adriatico after a tactical finale and a powerful sprint in Martinsicuro. A small group of favourites broke away in the final kilometres, where Van der Poel timed his sprint perfectly to take the victory. Giulio Pellizzari finished second and moved into the overall race lead after gaining valuable bonus seconds
